Understanding Pressure Ulcers

Understanding Pressure Ulcers

Pressure ulcers have several names, such as bedsores, pressure sores, and decubitus, but regardless of what we call the phenomenon, the problem is the same: When an area of skin has been subjected to prolonged pressure or friction for an extended period, it results in a lack of oxygen in the tissue, and pressure ulcers develop. It is often seen over bony prominences such as the sacrum, ischial tuberosity, greater trochanter, lateral malleoli, and heels. The underlying subcutaneous fat tissue and muscles are more sensitive to pressure than the overlying skin. Therefore, tissue necrosis develops in depth first, before the skin shows redness and ruptures.

How do pressure ulcers develop?

The development of pressure ulcers depends partly on the pressure applied, partly on the duration the tissue has been exposed to pressure, and partly on the location of the pressure and the patient's general health. Causes of pressure ulcers can be divided into internal and external factors. Most factors that are related to the individual are the internal factors. The external factors are related to the external environment and include pressure, shear (traction), and friction. A common risk factor for pressure ulcers is immobility.

Mattresses for the care sector - ZiboCare

Different types of mattresses for individual challenges

ZiboCare offers the possibility to reduce external factors that can contribute to the development of pressure ulcers through various aids and mattress types. The aim is to enable professional staff (with the right advice and support) to choose the solution and surface that best suits the individual. At the same time, consideration is given to the individual's internal factors that can contribute to the development of pressure ulcers.

The first step is to conduct a risk assessment. The most commonly used in Denmark is the Braden score, which can easily provide an indicator of the patient's risk of developing pressure ulcers. If the patient already has a pressure ulcer, it is important to determine its stage. The above tools are available from ZiboCare as a pocket card that guides you through the assessment.

Based on the risk assessment or pressure ulcer category, the right product can be chosen to help select the most suitable surface for the patient, based on the challenges the patient faces. Some patients have challenges that prevent them from lying on a standard mattress.

At ZiboCare, we find that a targeted solution for patients who, for example, are sensitive, have slow-healing wounds, or experience pain, can reduce their challenges and improve their quality of life. At the same time, a good solution for the patient can provide a better working environment for staff, reduce workload, and free up or save resources.

Equipment-related pressure ulcers

Sometimes, pressure ulcers develop due to equipment used by the patient, such as CPAP masks, oxygen catheters, or drains. A COPD patient sitting with their elbows pressed hard on the table to breathe can also develop pressure ulcers due to the static pressure between the elbow and the tabletop. Pressure pads can be placed between the affected area and the equipment to help reduce the risk of developing pressure ulcers.
